Annual Enrollment for OGB Participants

Blue Cross and Blue Shield of Louisiana proudly serves the state of Louisiana employees, retirees and dependents. During annual enrollment, we'll continue to offer the HMO plan administered by Blue Cross.

If you're already a Blue Cross member and want to keep this coverage, you don't need to do a thing!

To sign up or switch to Blue Cross, follow the instructions below:

• Ready to Enroll?
• Active employees can go to their agency’s HR Department to complete a GB-01 form.
• Retirees can go to their former agency's HR Department or to an OGB Customer Service office to complete a GB-01 form.
• Retirees can also enroll by sending a letter to OGB stating that they wish to change to the HMO plan administered by Blue Cross. Retirees should be sure to include their name, Social Security number and daytime phone number with the area code.The letter must be signed by the retiree, dated and mailed to:
