InHealth: Blue Health Services
Disease Management for OGB Members
Blue Cross and Blue Shield of Louisiana offers InHealth: Blue Health Services - a disease management program to help you if you have one or more of the following five health conditions: asthma, COPD, coronary artery disease, diabetes and heart failure. This program replaces the Living Well Louisiana (LWL) program.
At no additional cost to eligible members, the program offers you health coaching, prescription incentives, educational materials and caring support.
How to Join
This program is part of your health plan as long as Medicare is not your primary coverage. Once enrolled, you must speak with a Blue Cross Health Coach at least once every three months (or 90 days) to remain an active participant in the program.
To enroll or confirm your enrollment, call a Blue Cross Health Coach toll-free at 1-800-363-9159. Please log in and complete an online questionnaire so that your call with your Health Coach will better meet your needs.

Can You Participate in the Program?
- Are enrolled in the PPO, HMO, or Consumer Driven Health Plan (CDHP) health plan;
- Do not have Medicare as primary health coverage;
- Have been diagnosed with one or more of these ongoing health conditions:
- Diabetes
- Coronary artery disease
- Heart failure
- Asthma
- Chronic obstructive pulmonary disease (COPD)
What Can the Program Do for You?
- Learn more about your condition and how it affects you.
- Find out how to work with your doctor to manage or improve your health.
- Understand more about the medicines you take and why you take them.
- Receive health information that will help you understand, manage, and improve your condition.
What is a Health Coach?
Our health coaches are Blue Cross nurses or healthcare professionals who:
- Give you individual support and attention;
- Help you set healthcare goals;
- Assist with coordinating your care;
- Serve as your advocates and advisors;
- Give you important health information;
- Help you find qualified physicians, and
- Reduce the barriers to good health outcomes.
How Can the Program Save You Money on Prescriptions?
- Pay only $15 for brand-name drugs, when a generic is not available and $0 for generic drugs for a 31-day supply of covered drugs.
- Covered drugs include certain drugs specifically prescribed for treating diabetes, coronary artery disease, heart failure, asthma, and COPD.
