Annual Enrollment for OGB Participants

Blue Cross and Blue Shield of Louisiana proudly serves the state of Louisiana employees, retirees and dependents. During annual enrollment, we'll continue to offer the HMO plan administered by Blue Cross.

If you're already a Blue Cross member and want to keep this coverage, you don't need to do a thing!

To sign up or switch to Blue Cross, follow the instructions below:

• Ready to Enroll?
• Active employees can go to their agency’s HR Department to complete a GB-01 form.
• Retirees can go to their former agency's HR Department or to an OGB Customer Service office to complete a GB-01 form.
• Retirees can also enroll by sending a letter to OGB stating that they wish to change to the HMO plan administered by Blue Cross. Retirees should be sure to include their name, Social Security number and daytime phone number with the area code.The letter must be signed by the retiree, dated and mailed to:

OGB awards HMO health plan contract to Blue Cross

The Office of Group Benefits (OGB) awarded Blue Cross and Blue Shield of Louisiana the contract to administer its HMO plan for the state of Louisiana employees and retirees, effective Jan. 1, 2012.

Blue Cross was awarded the contract in a competitive bid process mandated by state law. Blue Cross has administered the program for the state from July 2010 through the present.

The plan’s benefit structure will remain unchanged.

Last year, Blue Cross enrolled more than 150,000 new OGB members, creating more than 200 new jobs in Louisiana to serve these members.

As part of Blue Cross’ ongoing quality improvement process, the company commissioned an independent national research survey in early 2011 to measure satisfaction among OGB members. The results showed that more than 75 percent of those members were highly satisfied overall with their Blue Cross experience, and more than 85 percent were highly likely to choose Blue Cross again.

Coverage for Influenza (Flu) Vaccinations

OGB's HMO health plan (administered by Blue Cross) covers influenza vaccinations (flu shots) administered at a pharmacy, health fairs, job sites, and physicians' offices. The claim form to be used is determined by the location where the flu shot is received. Claims will be processed as follows:   

• Flu shots rendered at a pharmacy:  Blue Cross pays billed charge at 100%.  Members pay up front and file a claim using OGB's Flu Vaccination claim form.

• Flu shots rendered at health fairs, job sites, etc.:  Blue Cross pays billed charge at 100%. Members pay up front and file a claim using BCBSLA 1500 claim form, not OGB flu vaccination claim form.

• Flu shots rendered by an in-network provider at the provider's office:  Blue Cross applies the in-network allowable and pays 100%.  Provider files a claim using BCBSLA 1500 claim form. 

• Flu shots rendered by an out-of-network provider at the provider's office:  Blue Cross applies the in-network allowable and pays 100%. Provider files a claim using BCBSLA 1500 claim form.
