With a Personal Health Record (PHR) you can track your own medical history and access it at the click of a button. It is useful to share with new doctors or in emergency situations.

Blue Health Record
Your Blue Health Record is based on past claims reported to Blue Cross over the past three years. It is a record of your care and a great supplement to your PHR.

Blue Health Record Features

• Can be accessed by patient or physician (if allowed)
• Gives physicians seeing you for the first time important information about your health
• Shows each “episode” of care and the treatment received over a three-year period
• Lists the prescriptions you are currently taking and have taken as a covered member and gives contact information from prescribing physicians
• Limited to claims that have been paid by us. You should provide additional information to your doctor as needed.
• Not all AccessBlue users have Blue Health Record

Members and providers must be registered for our online services to access Blue Health Records (if available).
