What is a Personal Health Assessment?
A Personal Health Assessment (PHA) is a questionnaire that asks about your health and lifestyle to help determine what health risks you may have.
Why should I take a PHA?
A PHA will help you see your overall health by asking you about all aspects of your life, not just your physical health. With your results, you can start to learn more about possible risk factors, find ways to reduce those risks and know what to talk to your doctor about in the future.
I’m healthy. How can a PHA help me?
Even healthy people can benefit from a PHA. You will find questions about stress, safety practices, family medical history and more. Your results are designed to show you what to look out for in the future, not just problems you may be experiencing today.
What does Blue Cross do with the information I enter into the PHA?
The answers you provide are protected as personal health information. If you have a chronic condition or other risk factors that could benefit from personal support and attention, the information from your PHA may be provided confidentially to the Health Services unit, and a Blue Cross nurse may contact you to discuss your healthcare goals.
What is a Campaign Code and where do I find one?
When you click on “yourPHA” from AccessBlue, you will be asked to provide a Campaign Code before taking the PHA. If you do not already have a code, you can use the one provided in AccessBlue or you can call 1.800.495.2583 to receive a code from Customer Service.
