Our Care Management Department is committed to providing a wide range of wellness information and preventive services. Members can find solutions to manage their wellness or chronic condition using information from the newsletters, wellness reminders, educational materials and online services.

Information gathered through our online tools may be shared with our in-house nurses to assist you in managing your health. However, all your information is confidential, and it will not be used for setting premiums.

Have you taken a Personal Health Assessment (PHA)?
The best way to help our nurses know if you will benefit from our care management services is to take a PHA. This tool will review your current and past health issues and habits to determine your overall state of health. Login to your online account and click Personal Health Assessment to begin.

What can you do to manage your health?
Blue Cross and HMO Louisiana provide information on managing a number of conditions. Login to your online account and click Health Topics to view information on conditions such as: 

  • Diabetes
  • Heart Disease
  • Congestive Heart Failure
  • Asthma
  • Chronic Lung Disease

For general information on health and to compare providers, login to your online account and click Healthcare Advisor.

How can you keep track of your condition?
You can track your care using our member-only Go-To Guides.* These interactive workbooks will help keep you focused on your health goals. These workbooks include information on Diabetes, Asthma, Congestive Heart Failure, Coronary Heart Disease and Chronic Lung Disease. Login to your online account and click Health Workbooks to access these tools.

You can also take advantage of informative videos on various lifestyle topics. Members can access these videos by taking your Personal Health Assessment through your online account.

How can I contact a nurse?
If you wish to contact a Care Management nurse to inquire about our programs, please call the Case and Disease Management Unit at 1.800.317.2299.

  • Continuity of Care Form
    If your doctor leaves our networks while you are receiving care for certain conditions, he or she may request you continue care at a higher network level of benefits for a limited time.
