What if I or a member of my family needs emergency care?
What customer services are available to me online? What are the fax numbers to the Blue Cross and HMO Louisiana Claims areas? Where can I get and/or mail a Blue Cross or HMO Louisiana claims form? My claim was returned as being investigational. What does investigational mean? I did not receive my claim payment. Can you re-issue the claim payment? How can I get prescription drug information? What is the amount of my deductible? Can I change my primary care physician (PCP)? How can I change my address with Blue Cross and/or HMO Louisiana? How can I terminate my Blue Cross or HMO Louisiana policy? How can I get a new Blue Cross or HMO Louisiana ID card?
Members under an employer-sponsored plan cannot access these services unless your employer has elected to participate. Primary cardholders can access member maintenance information for themselves and those covered under their plan.
Can I file a claim online?
