Matching Gift Program

Full-time employees, retirees and board members of Blue Cross and Blue Shield of Louisiana can increase the value of their donations to charitable and other organizations through a matching gift from the company.

Eligible Institutions and Amounts

Program provides dollar-for-dollar matching contributions to 501(c)(3) or other eligible nonprofit organizations
Requests must meet BCBSLA philanthropy criteria
Minimum gift of $100, with maximum match of $1,500

Eligible Donors

BCBSLA board members
Full-time employees of BCBSLA
BCBSLA retirees

How to Participate

Complete top portion of the Matching Gift form [F-PDF] Updated 02/2009
Send form with a personal check to the eligible organization
A copy of both the form and check should be sent to BCBSLA Community Relations
Eligible institution should then complete bottom portion of the form, sending the form, copy of original check and copy of the organizaton's IRS tax exemption letter to BCBSLA Community Relations (address below)
Matching gift will be made upon completion of BCBSLA review to verify eligibility
