2013 Benefits Information Now Available

Attention CDHP Bancorp Bank HSA Account Holders: Enjoy free, secure and convenient access to your statements by signing up for online statements. Simply log into your MySmartSaver HSA account at www.mysmartsaver.com and select online/paperless statements. If you prefer paper statements, your account will be billed a small monthly fee of $1.25 for paper statements. Disease Management Enrollment

If you were enrolled in the Living Well Louisiana (LWL) Disease Management program with Nurtur prior to January 1, 2013, we have been trying to reach you so we can enroll you in the In Health Disease Management Program.

If you would like to remain an active participant in the Disease Management program offered by OGB, you must speak with a Blue Cross Health Coach as soon as possible. Any member previously enrolled in the Living Well Louisiana program who has not enrolled in the Blue Cross In Health Disease Management program by March 31 will no longer be eligible to receive the zero dollar generic drug copay and $15 brand name drug copay incentive.
