Stay in Your Network

Control costs by staying in your network. Your plan includes both in-network and out-of-network coverage. There are two in-network tiers: Enhanced Tier 1 and Tier 1.

  • Enhanced Tier 1 – Provides the most value and includes FMOLHS providers. You will pay the lowest copayment when you see a PCP or Specialist in Enhanced Tier 1.
  • Tier 1 – Includes the remaining in-network providers that are not part of the FMOLHS system. You will pay more to see a PCP or Specialist in Tier 1 than in Enhanced Tier 1, and you will pay the most when you visit an out-of-network doctor or hospital.

Be proactive about staying in your network by visiting to find a doctor or hospital in your network. The Precision Blue network is anchored by the Franciscan Missionaries of Our Lady Health System (FMOLHS). Other providers in the network include Woman’s Hospital and Louisiana Women’s Healthcare.

Explanation of Benefits

After each visit to a doctor or hospital, we will send you an Explanation of Benefits (EOB). It is a summary of the charges for the care or services you received. It shows what we cover and what the provider charged or what the service cost. It is not a bill. You may choose to go paperless and get your EOBs online by logging in at and choosing "Account Preferences."
