Live Better Louisiana gives you access to a wide range of healthful activities—some of which may even be suggested in your personal action plan. Blue Cross and Blue Shield of Louisiana also brings OGB plan members a number of wellness-related Discounts and Deals.

Disease Management: A program at no additional cost to eligible members that offers health coaching, prescription incentives, educational materials and caring support. Conditions addressed are asthma, COPD, coronary artery disease, diabetes and heart failure. Learn more on our Managing Your Care page.

Case Management: With our voluntary Case Management program, we want to give you the power to effectively manage your own healthcare. At the same time, we want to support all your values and preferences. For eligible members, Case Management helps you become a true partner in your healthcare and not just a receiver of services. Learn more on our Managing Your Care page.

Diabetes Prevention Program: OGB members who are diagnosed with prediabetes or at high risk for type 2 diabetes may be eligible for Omada, a 16-week diabetes prevention program from Blue Cross and Omada Health. The Omada program is designed to help individuals, including those with prediabetes, lose weight and make healthier choices. Combining a professional health coach with an understanding and supportive small group, a flexible online format and a growing pool of healthy resources, Omada is a profound, lifestyle-changing experience.

Primary Care Referral: Studies show that building a long-term relationship with a single doctor can improve your health. Your primary care physician (PCP) is your healthcare team quarterback, coordinating your care and helping ensure you don't get unnecessary tests or treatments. OGB members who don't already have a PCP will get help choosing one from the appropriate Blue Cross network.

Diabetes Care: OGB members diagnosed with diabetes may be eligible for a glucometer and test strips from Abbott Diabetes Care at no out-of-pocket cost to help manage their diabetes. You don’t have to enroll in any additional programs, but you must be participating in the InHealth: Blue Health Diabetes program to qualify for test strips. Follow these instructions to claim your no out-of-pocket glucometer and test strips.
