Close to half of everyone in the United States lives with “chronic” or long-term health conditions, such as diabetes, heart disease or kidney disease. That’s more than 145 million people.
Chronic diseases represent more than 75% of all healthcare costs in America. Blue Cross and Blue Shield of Louisiana is leading the way to help primary care doctors redesign healthcare for patients. We want to get better results for patients with long-term diseases.
Patients who receive their primary care services from a Quality Blue Primary Care (QBPC) physician will have the benefit of a full care team—both their own doctors and the Blue Cross staff—working together to improve their health. Blue Cross will provide QBPC doctors with the support and resources they need to work closely with their patients with certain chronic conditions. Patients will benefit from improved care coordination because their doctors will have actionable data right at their fingertips to make the most of office visits and promote exceptional health outcomes. And Blue Cross will reward doctors for giving care that produces better health. Everyone wins.
Our primary care system must be transformed to meet current and future demands from the challenges it faces. These challenges include widespread chronic diseases, a rapidly aging population and a greater demand for primary and preventive care. The need for a well-organized and effective primary care system is clear.
The Patient-Centered Medical Home (PCMH) is a model designed to replace a fragmented, disease-focused approach to care with organized, proactive care that works to prevent disease and preserve or restore health.
The medical home model is also meant to promote high-quality, personal and effective care.
Quality Blue Primary Care uses many of the same principles as PCMH, and is also designed to improve care coordination and quality. But, while the PCMH model requires a significant time and resource commitment by the practice, QBPC will be put in place with the professional clinical support, technology and guidance of Blue Cross and Blue Shield of Louisiana. We provide the framework and resources to let each practice reach the best possible population-based team approach to care with as little disruption as possible.
Quality Blue Primary Care also aligns Blue Cross with its network of primary care practices to manage patient populations actively. It provides standardized support and services to primary care practices and members within the network.
Last but not least, QBPC provides physicians with continuing medical education (CME) to improve both the physician’s and the clinical staff’s knowledge of how to provide the highest-quality care possible.
To implement QBPC, Blue Cross and Blue Shield of Louisiana is targeting primary care physicians in the network – family medicine, internal medicine or general practice – that have high volumes of patients who are Blue Cross members with one or more chronic conditions. Over the next three years, Blue Cross intends to engage the majority of primary care physicians in high volume practices.
